The purpose of this Indian Health Service cooperative agreement is to provide diabetes treatment or prevention activities/services for American Indian and Alaska Native communities. Activities/services will be aimed at reducing the risk of diabetes in at-risk individuals, providing high quality care to those with diagnosed diabetes, or reducing the complications of diabetes.

Eligibility: 

Federally-recognized Indian Tribe; Tribal organization operating an Indian health program with the HIS; urban Indian organization operating a Title V urban Indian health program; IHS facilities
